Yes it has happened before, people got DQ-ed for attacking managers with weapons.

It's more about using a weapon in a regular match than attacking the manager though I believe.

I'm not saying it's never happened but I want to see if the example you find is one that was universally criticized as a bad ref call

Bc that would be fucking atrocious, the ref should have instantly kicked Dom out and threw the chair away himself instead of standing there waiting to see what Zelina would do
